I__________________________________give my consent for my name, telephone number, address and email address to be held on a membership list by the committee of Lapworth Ladies Supper Club.

This information will only be used by the club for purposes in connection with the running of the club, which includes communicating by post, telephone and email.

I understand that none of my data will be passed onto any third parties at any time.

The electronic membership list will be stored on a password protected computer.

The membership list, will be updated by the end of December each year and member’s data who are not re-joining from the previous year, will be deleted.

Your details can be removed from our records at any time of the year, within 28 days of a written request to cancel your membership to the club secretary.

Please note it is expected that attendees have been vaccinated against covid, will not attend if they have any symptoms of illness and will abide by any current government covid regulations or requests by venues (eg wearing of a facemask when not seated for dining)
